2020-2021 Enrollment Information

We highly encourage parents of new students to use LAUSD's online Pre-Enrollment System online at https://enroll.lausd.net 

If you are unable to complete the forms using the online system, the following information is provided for you.

STEP 1: COMPLETE YOUR 2020-2021 STUDENT PRE-ENROLLMENT PACKET

COMPLETE THE ATTACHED FORMS (these are fillable PDF forms that you can type, save, and print):

• LAUSD Student Enrollment Form
• LAUSD Student Emergency Information Form
• LAUSD Student Housing Questionnaire

PROVIDE VERIFICATION DOCUMENTS FOR:

• Proof of Residence
  • Acceptable documents: utility service contract, property tax payment receipts, rental lease agreement, official government mail, current pay stub, voter registration
• Proof of Student’s Age
  • Acceptable documents: birth certificate, baptismal certificate, passport, court order, health office/vital statistics record of birth certificate, dated Department of Public Social Services (DPSS) letter verifying birthdate and an explanation of how this was verified
• Identity Verification of Parent/Guardian/Educational Rights Holder/Caregiver (“parent”)
  • Acceptable documents: student’s birth certificate, baptismal certificate, adult’s government-issued photo identification (driver’s license, department of motor vehicle identification card)
• Proof of Immunization
  • Please find enclosed for your reference, the Parents’ Guide to Immunizations Required for School Entry
• IF AVAILABLE/APPLICABLE PLEASE ALSO PROVIDE COPIES OF:
  • Transcripts or recent grade reports. Copy of most recent Individual Educational Plan (IEP)* or Section 504 Plan £ Department of Children and Family Services (DCFS) 1399 Form or minute order

STEP 2:  Call EVHS at 818-753-4400 to schedule a time to return the documents.  Please note that the school office may not be open during the summer as Summer School is being done via Distance Learning.  You may also email our office staff using the Staff link on the right and they will contact you when the school office is open.

• La forma de Información de Emergencia para el estudiante de LAUSD
• El Cuestionario de LAUSD de Vivienda del Estudiante

PROPORCIONAR DOCUMENTOS DE VERIFICACIÓN PARA:

• Verificación de Residencia
  • Documentos aceptables incluyen: Contrato de Servicio de Utilidades, Recibos de pago de sus impuestos a la propiedad, contrato de renta, correo oficial de gobierno, talón de cheque reciente, registración de votante
• Verificación de Edad del Estudiante
  • Documentos aceptables incluyen: Acta de nacimiento, certificado de bautizo, pasaporte, orden de la corte, récord del certificado de nacimiento de la oficina de salud/estadísticas vitales, carta fechada del Departamento de Servicios Sociales Publico (DPSS) verificando la fecha de nacimiento y una explicación de cómo esto fue verificado
• Verificación de identidad del padre/guardián/poseedor de derechos educacionales/Cuidador
  • Documentos aceptables incluyen: acta de nacimiento del estudiante o acta de bautizo y una identificación gubernamental con foto (por ejemplo, licencia de manejo y tarjeta de identificación del departamento de motores y vehículos)
• Verificación de Vacunas
  • Se adjunta para su referencia, la Guía de Padres a las vacunas requeridas para el ingreso escolar
• SI ES DIPONIBLE / APLICABLE, TAMBIÉN PROPORCIONE COPIAS DE:
  • Expediente o reporte de calificaciones reciente. Copia del Plan Individualizado Educacional más reciente (IEP) o Plan de Sección 504. Para niños en Hogares de Guarda, la forma 1399 del Departamento de Servicios Sociales para niños y familias (DCFS) o la orden de minuto.

PASO 2: Llame a EVHS al 818-753-4400 para programar una hora para devolver los documentos. Tenga en cuenta que la oficina de la escuela no puede estar abierta durante el verano ya que la escuela de verano se realiza a través de la educación a distancia. También puede enviar un correo electrónico al personal de nuestra oficina utilizando el enlace del Personal a la derecha y se comunicarán con usted cuando la oficina de la escuela esté abierta.
